This occurs when you have incorrectly answered CareTeam registration security questions.

DocbookMD is unable to see the questions and answers to your security questions. These questions and answers are generated by a third party, which pulls it's data from your credit history and public records. You must answer the security questions properly, in order to be given access to DocbookMD's secure messaging system. You are signing a HIPAA compliance agreement. We must confirm your identity.
